About the Role As a Nurse Practitioner, you will conduct thorough health assessments during visits that may last up to an hour. These assessments offer crucial insights into a member's overall health, frequently overlooked in traditional office settings. Your main responsibilities will include: Performing adult and geriatric assessments, including health history reviews, diagnosis and treatment planning, health education, and case management referrals, as well as detailed documentation. Administering and ordering necessary point-of-care and lab tests. Collaborating with physicians, case managers, social workers, family members, and multidisciplinary medical staff as needed. Working with Primary Care Physicians to enhance patient education and ensure seamless follow-up care. Providing services in members’ residences, including homes or Skilled Nursing Facilities. Benefits of Working at Matrix Work-Life Balance: Enjoy a flexible schedule, providing assessments from Sunday to Saturday, 8 am to 7 pm, with full-time providers typically working Monday to Friday, 8:00 am to 5:30 pm. Competitive Pay: Earn an annual salary ranging from $105,500 to $168,400 based on experience and market conditions. Incentive Program: Access uncapped quarterly incentives. Qualifications Master's Degree or equivalent experience with successful NP licensure. Current NP licensure in the state of practice, with prescriptive authority or the capability to obtain it. AANP, AACN, or ANCC board certification in relevant specialties (Adult, Adult Gerontology Primary Care, Family, etc.). Current BLS, ACLS, or CPR certification. At least one year of NP experience. Able to travel up to 80-90% of the time for extended periods, including overnight and weekend travel. Valid state driver's license and automobile insurance coverage. Proficiency with medical record software and exceptional communication skills.
