RN Case Manager - Hospice We are seeking an experienced RN Case Manager - Hospice to provide compassionate, high-quality care to hospice patients. Position Details Visit up to 5 patients per day Schedule: 3-5 days per week Days: Monday through Friday Pay: $41.00-$44.00 per hour Mileage reimbursement: One-way mileage paid based on the longest distance traveled Responsibilities Provide skilled nursing visits to hospice patients in their homes or care settings Assess patient condition and develop/update care plans Coordinate care with physicians, families, and interdisciplinary team members Educate patients and families on hospice care and symptom management Complete accurate and timely documentation VIP Supreme Staffing LLC is an EEO Employer - M/F/Disability/Protected Veteran Status
