RN Hospice Case Manager - Oak Brook, IL Protouch Staffing is currently seeking a dedicated RN Hospice Case Manager to join a compassionate care team in Oak Brook, Illinois. This role is ideal for Registered Nurses with experience in home health or visiting nurse services who are committed to delivering high-quality, patient-centered hospice care in a home setting. Position Details Job Title: RN Hospice Case Manager Location: Oak Brook, IL Work Setting: Onsite / Home Visits Start Date: ASAP Schedule: Monday-Friday Shift: Day Shift (8:00 AM - 5:00 PM) Hours: 40 hours per week Key Responsibilities Manage and coordinate hospice care for assigned patients in a home setting Perform comprehensive patient assessments and develop individualized care plans Provide direct nursing care including medication administration and symptom management Coordinate care with physicians, hospice team members, and caregivers Educate patients and families about disease progression, care plans, and symptom management Maintain accurate clinical documentation according to hospice guidelines Ensure quality, compassionate, and patient-focused end-of-life care Qualifications and Requirements Active Registered Nurse (RN) license Prior experience in home health or visiting nurse roles required Experience in hospice or palliative care preferred We also have additional RN opportunities available across various specialties and locations.
