The RN will be responsible for conducting in-home patient assessments, performing skilled nursing visits, and developing individualized care plans in collaboration with the interdisciplinary care team. This position plays a critical role in ensuring high-quality, patient-centered care delivered in the comfort of our clients' homes. Key Responsibilities: Perform comprehensive patient assessments in the home environment, including physical, psychosocial, and environmental assessments. Develop and implement individualized care plans based on patient needs, physician orders, and best practices. Provide direct nursing care, including wound care, medication administration, catheter care, injections, and other skilled nursing tasks. Monitor patient condition and progress; communicate any changes to the physician and care team. Educate patients and caregivers on disease process, medications, self-care techniques, and health maintenance. Accurately document all assessments, interventions, and communications in the electronic medical record (EMR) system in a timely manner. Collaborate with multidisciplinary team members, including physicians, physical therapists, occupational therapists, and social workers. Ensure compliance with all state and federal regulations and agency policies. Participate in case conferences, staff meetings, and ongoing training as required. Qualifications: Current Registered Nurse (RN) license in MA Minimum 6 months to 1 year of nursing experience, preferably in home health, community health, or acute care settings with assessments.
