Position Title: Registered Nurse (RN) Department: Nursing Reports To: Director of Nursing / Nurse Manager Overview We are seeking a compassionate and skilled Registered Nurse (RN) to join our healthcare team. In this role, you will assess, plan, implement, and evaluate patient care from admission through discharge. The ideal candidate is detail-oriented, dependable, and committed to providing high-quality, patient-centered care. Pay rate varies on experience. Key Responsibilities Patient Assessment: Perform comprehensive initial and ongoing assessments, identifying physical, functional, psychosocial, and cognitive needs. Care Planning: Develop and implement individualized care plans; adjust as needed in collaboration with physicians, families, and the care team. Clinical Interventions: Provide skilled nursing care including wound care, medication administration, IV therapy, injections, and emergency response. Monitoring & Reporting: Monitor vital signs, track health changes, document all findings, and promptly communicate concerns to the healthcare team. Documentation: Complete accurate and timely electronic or paper charting in accordance with facility standards. Patient & Family Education: Provide clear education on diagnoses, care plans, medications, and discharge instructions. Care Coordination: Collaborate with therapists, LPNs, aides, and interdisciplinary team members to ensure continuity of care. Supervision: Offer guidance, instruction, and oversight to LPNs and home health aides as needed. Safety & Compliance: Follow all policies, procedures, infection control protocols, and regulatory requirements to maintain a safe environment. Qualifications Graduate of an accredited School of Nursing (BSN preferred). Active RN license in the state. Current BLS/CPR certification (ACLS may be required depending on department). Previous clinical experience preferred (6 months-2 years depending on setting). Strong assessment, communication, and critical-thinking skills. Ability to work independently and as part of a team. Medicaid and Medicare experience a plus. Physical Requirements Ability to stand, walk, lift, bend, and move for extended periods. Able to lift/push/pull up to 50 lbs when transferring patients or equipment. Visual and auditory acuity to assess patient needs. Exposure to common clinical conditions, including infectious materials. Position Purpose The RN provides high-quality nursing care by assessing, planning, implementing, and coordinating patient care throughout the continuum-from admission to discharge.
