Position Summary: At Eventus WholeHealth, we are passionate about delivering exceptional healthcare services to adults in skilled nursing and assisted living facilities. Join our collaborative network of health professionals, including Physicians, Nurse Practitioners, Physician Assistants, Medical Assistants, and dedicated Support Staff, as we strive to improve the quality of care for our patients. Essential Duties and Responsibilities: As an integral member of our team, you will provide vital primary care services that align with Eventus WholeHealth protocols, focusing on family medicine, geriatric medicine, and internal medicine specifically designed for long-term care environments. Quality/Compliance: Review and sign MARs as needed and efficiently address all pertinent orders related to Primary Care within the facility or within 24 hours, including phone orders, DME orders, Death Certificates, Plans of Care, and hospice orders to meet patient care needs. Complete documentation and submit billing records in accordance with Eventus WholeHealth policies, ensuring facilities receive a hard copy of your electronic documentation within 48 hours post-service for inclusion in the patient's medical record. Engage in transparent communication with facility staff and administration, providing relevant feedback on medications and patient care to foster collaborative efforts. Daily meetings with the designated facility care coordinator before and after patient visits are expected. Maintain awareness of and comply with Eventus WholeHealth standards, policies, and all relevant legal regulations governing healthcare delivery. Adhere to all ethical standards of practice. Uphold patient confidentiality tirelessly, employing secure methods for communication, documentation, and electronic records management as per HIPAA regulations. Consult with your clinical supervisor prior to declining any requests or recommendations from families or facility staff concerning resident care. Qualifications/Education: A Master's degree in Nursing from an accredited institution, alongside a valid medical practice license in your residing state, with all other necessary certifications being current and valid. Completion of an accredited Physician Assistant educational program recognized by the National Commission on Accreditation of Allied Education Programs. Successful completion of the national certification examination from the National Commission on Certification of Physician Assistants. Possess a valid Physician Assistant license in your state of residence. Specialization in geriatric, family, or psychiatry is preferred.
