Details Home health setting serving adult and geriatric patients in the Charlotte, NC area Common diagnoses and care needs include chronic diseases, wound care, wound vac, IV therapy, and pleurx management Expected caseload of approximately 6 visits (points) per day Day shift schedule, Monday–Friday, 8:00 AM–5:00 PM No weekend rotation, no on-call requirements, and no floating requirements No holiday coverage required EMR: Homecare Homebase (experience required, no exceptions) Requirements Minimum 1 year of home health physical therapy experience required; most recent experience must be within the last year Active state PT license required (license required if applicable) Current BLS certification required Homecare Homebase EMR experience is required — no exceptions OASIS documentation experience required Proficiency in IV therapy, pleurx, wound vac, wound care, patient assessment, disease teaching, and maintenance notes required Experience with geriatric and adult populations and chronic disease management required Valid driver's license and proof of auto insurance required Last 4 digits of SSN and full date of birth required for consideration Minimum 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN Additional Information Daily responsibilities include skilled PT evaluations, treatment, OASIS assessments, disease management education, and maintenance note documentation Therapists work independently in the field with support from departmental leadership 5-day orientation (NEO) provided at the start of the assignment Grey scrubs required for therapy staff Free parking available at the home unit location Candidates must reside at least 50 miles from the home unit to be eligible — local candidates are not accepted First-time travelers will be considered if they have solid home health experience Candidates who have worked a permanent position at an AHSE facility must have been separated for at least 1 year before being eligible RTO requests require manager approval American Traveler Job ID #P-685775.
