Position: Nurse Practitioner (NP) Mobile/Travel Role Location: ISNP San Fernando Valley (Burbank Santa Clarita West Hills) Start Date: ASAP Duration: 6 months with extension options Pay: Bid Overview: Seeking an experienced Nurse Practitioner to provide geriatric primary care to patients in their homes and facilities throughout the San Fernando Valley. This is a mobile/travel NP role focused on value based care. Schedule: Monday Friday 8:00 AM 5:00 PM No on call Patient volume: 5 7 per day Requirements: Active CA NP license Board Certified (BC) BLS & DEA Minimum 1 year geriatric primary care experience Experience visiting homes/facilities as a mobile NP Must have knowledge of HEDIS and Value Based Care (VBC) Details: W2 with benefits offered EMR: Athena Physician support available by phone/virtual by Jobble
