As a Skilled Vocational Nurse, you will be a vital part of our team, providing direct care to participants in the WelbeHealth clinic. The LVN/LPN will play a key role in ensuring timely and quality nursing care, assisting with clinic overflow, monitoring vital signs, conducting point-of-care testing, and managing medication administration. Key Responsibilities: Deliver and thoroughly document wound care in accordance with your scope of practice and as directed by the Primary Care Provider (PCP). Perform and document necessary procedures, including medication management and administration, while adhering to your scope of practice and following PCP guidance. Administer medications to participants during clinic visits, ensuring they understand their medication regimens. Communicate with providers regarding any changes to medication orders following same-day procedures and oversee these adjustments as instructed by external providers. Conduct medication reviews and reconciliations for new participants during their Welbe Welcome Day visits. Carry out pre and post-operative clinical assessments, clarify instructions from specialists, and follow up with participants post-procedure; escalate any questions or concerns to the RN Case Manager or PCP as needed. Adapt to changing business needs, demonstrating flexibility in your assignments. Qualifications: Graduate of an accredited nursing program with a valid, unencumbered LVN/LPN license. At least one year of nursing experience in a clinical setting, particularly with frail or elderly patients. Strong clinical knowledge and skills necessary to effectively care for elderly participants and manage complex health situations. Highly motivated and self-directed, capable of making informed decisions in emergency situations and thriving in a dynamic environment. Exceptional communication, clinical, and organizational skills, especially when interacting with seniors, their families, and healthcare team members. Well-organized, with the ability to multi-task effectively for various patients. CPR and First Aid certification must be obtained within 180 days of hire; certification upon hire is preferred.
