Job Description & Requirements Specialty: Family Practice Discipline: Nurse Practitioner Start Date: 05/01/2026 Duration: 28 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Start: May 2026 – Nov 30, 2026 Schedule: Mon–Fri, 8 AM – 5 PM Call: None  Overview PACE program serving complex elderly/disabled patients. Smaller patient panels (~70) with a focus on comprehensive, team-based care.  Responsibilities Primary care for PACE participants Manage chronic & acute conditions Interdisciplinary team collaboration Clinic, home, nursing home, or phone/in-basket coverage EMR: EPIC  Requirements Active MA license Flu vaccine required (no exceptions) Recent supervisor reference Travel, Lodging and Malpractice will be covered.
