Territory: Philadelphia, Montgomery, Bucks, Delaware & Chester Counties Position Summary A Medicare-certified, Pennsylvania-licensed home health agency is seeking a PRN Field RN to deliver skilled nursing care to patients throughout the greater Philadelphia region. The RN performs routine home visits, provides hands-on clinical care, assesses patient progress, and collaborates with the interdisciplinary team to support safe recovery at home. Key Responsibilities Provide skilled nursing visits per physician plan of care Complete ongoing assessments and monitor changes in condition Deliver wound care, medication management, and patient education Teach patients and caregivers home-based care techniques Communicate regularly with physicians and care team members Document care accurately and promptly in the EMR Qualifications Active PA Registered Nurse (RN) license Graduate of an accredited nursing program Home health experience preferred Strong assessment and education skills Ability to work independently in the field Valid driver's license and reliable transportation Coverage Area Serving patients in: Philadelphia • Montgomery • Bucks • Delaware • Chester Counties Position Details PRN / Per Diem Flexible scheduling Visits assigned by territory and RN availability Join a team committed to high-quality, patient-centered care-helping individuals recover, regain independence, and thrive in the comfort of home.
