As a Home Health Registered Nurse, you will deliver skilled and compassionate care directly in participants' homes, adhering to CMS guidelines and ensuring they have convenient access to essential health services. Collaborating closely with our interdisciplinary team, you will develop care plans that empower participants to live healthier, more independent lives. Your daily responsibilities will include: Performing home health nursing assessments to evaluate the needs for nursing equipment, including participant preferences and goals. Actively engaging in interdisciplinary team (IDT) meetings to co-create individualized care plans. Delivering and documenting nursing interventions in alignment with care plans, ensuring a healthy and safe environment, timely response to physician orders, and accurate medication administration, including tests and treatments. Educating participants, their families, and caregivers on disease management, self-care techniques, and preventive strategies related to their conditions. Managing home health scheduling in collaboration with the Home Health Manager or designated scheduling contact. Collaborating with physicians and fellow IDT members to ensure seamless transitions of care across various settings (hospitals, skilled nursing facilities, home, etc.). To qualify for this position, candidates must meet the following criteria: Must have graduated from an accredited school of nursing. An unencumbered RN License is required. A minimum of one year of nursing experience in a clinical setting with elderly or frail populations is necessary; experience in home health care is preferred. Reliable transportation is essential. CPR and First Aid certification must be obtained within 180 days of hiring and maintained as required; certifications upon hire are a plus.
