Join us in making a difference! Work Schedule: Monday - Friday, 8:00 AM to 5:00 PM CST or EST, including rotating on-call responsibilities. The Virtual Nurse Practitioner collaborates with physicians and interdisciplinary care teams to create personalized, comprehensive care plans tailored to each patient's unique needs while delivering clinical and supportive care. Key Responsibilities: Provide primary and urgent health care through telephonic and video interactions to patients enrolled in state Medicaid programs and other memberships. Develop and implement clinical care plans for adult patients facing various chronic and complex health conditions. Conduct thorough health assessments and physical examinations to create effective treatment plans aligned with patients' health goals. Identify and address gaps in patient care, ensuring comprehensive support. Promote patients' physical and emotional well-being in collaboration with an interdisciplinary team. Educate patients and their families on medication management, health conditions, and preventive measures. Ensure optimal patient care by coordinating with clinical teams and other healthcare providers in compliance with regulations. Document all patient interactions consistently, adhering to state regulations and company policies. Engage in ongoing education to maintain prescribing authority as permitted. Minimum Qualifications: Master's degree in Nursing. Active national NP certification. Current, unrestricted NP license in New Jersey. Valid compact RN license or compliance with state license requirements. At least 2 years of experience managing complex care cases. Familiarity with Electronic Medical Records (EMR). Preferred Qualifications: DEA registration or eligibility. Active Medicaid number in New Jersey. Experience in managing care for individuals with developmental disabilities and chronic illnesses. Compensation for this role ranges from $107,680 to $161,520, based on experience, education, and location.
