About the job Full-Time Hospice LVN (Valley area) Reliance Hospice and Palliative Care is seeking a compassionate Full-Time Licensed Vocational Nurse (LVN) to join our hospice team. This is a field-based position covering the San Fernando Valley area. Schedule & Compensation Position: Monday to Friday Weekend coverage: Candidates willing to cover weekends as needed will be strongly considered Rate: $35/hour or more, depending on experience Position type: Field-based role - candidates must be able to drive within assigned coverage areas Responsibilities Provide direct nursing care to hospice patients in private homes, assisted living facilities, board and care homes, and skilled nursing facilities under RN supervision Perform and document nursing interventions in accordance with the Plan of Care Assist with patient assessments, orders, and coordination of interdisciplinary visits Administer medications and treatments as ordered Monitor patient condition and report changes to the RN Educate patients and families on care needs and symptom management Triage calls as needed and attend death visits Maintain accurate, timely documentation Preserve patient dignity and comfort at end of life Make scheduled, requested, and Continuous Care visits as required Experience Requirements Experience working with Older adults, chronically or seriously ill patient, Hospice, home care, or community health settings Background in caregiving, case management support, or patient advocacy preferred Volunteer hospice or healthcare experience considered Qualifications Active California LVN license (required) Minimum 1 year of hospice or home health experience (required) Current CPR certification Valid California driver's license and proof of auto insurance Strong communication, documentation, and organizational skills Ability to work independently in home and community settings Empathy, emotional resilience, and comfort discussing sensitive topics Phlebotomy and/or IV certification (preferred) Multilingual candidates encouraged to apply Physical Requirements Ability to walk or stand for extended periods Ability to lift and transfer patients and equipment Frequent bending, kneeling, reaching, and crouching Adequate vision and hearing to perform LVN duties safely
