Responsibilities Provide skilled nursing care to patients in their homes, following individualized care plans and protocols. Utilize Kinnser software to document visits, update patient records, and coordinate care efficiently. Conduct comprehensive patient assessments and communicate changes in patient condition to the healthcare team. Educate patients and their families on health management, medication administration, and preventive care. Collaborate with interdisciplinary teams to ensure continuity of care and optimal patient outcomes. Adhere to all regulatory, compliance, and safety standards in home health care delivery. Requirements Current Registered Nurse (RN) license in the state of Michigan. Proven experience with Kinnser Home Health software is required. Minimum of 1-2 years of home health nursing experience preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills to provide education and support to patients and families. Reliable transportation and ability to travel throughout Redford and surrounding areas.
