Job Description & Requirements Specialty: Care Manager Discipline: RN Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Job Summary: We are seeking an experienced RN Case Manager to support patient care coordination and discharge planning in an acute care hospital setting . The RN will work closely with interdisciplinary teams to ensure efficient, patient-centered care and smooth transitions throughout the continuum of care. Key Responsibilities Coordinate and manage patient care plans and discharge planning . Collaborate with physicians, nurses, and multidisciplinary teams to ensure timely and effective care delivery . Assess patient needs and develop appropriate discharge plans . Facilitate communication between patients, families, and healthcare providers. Ensure compliance with hospital policies and case management standards . Maintain accurate and timely documentation .
