Position Summary This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center's strategic plan and participate in and advocate performance improvement/patient safety activities. To provide intermittent visits to the terminally ill patient, and palliative patient providing direct personal care and related duties to maximize quality of life under the direction of an RN and Clinical Supervisor. Essential Functions and Responsibilities of the Job 1. Provide direct personal care and related duties to terminally ill patients in a safe, appropriate and effective manner under the direction of an RN and in compliance with Hospice policies and procedures. 2. Communicate with patients, care givers and staff to assure timely and accurate relay of information to appropriate individual regarding date/time of planned visit. 3. Document patient care activities in a concise, appropriate and timely manner to assure completeness of the medical record per Medicare standards. 4. Be at work and be on time 5. Follow company policies, procedures and directives 6. Interact in a positive and constructive manner 7. Prioritize and multitask Qualifications Minimum Requirements Qualifications/Work Experience: Able to provide direct patient care such as transferring and positioning adult patients Minimum 2 years' experience in direct patient care as a Certified Home Health Aide
