Job Title: Registered Nurse (RN) - Case Manager Location: Summit, NJ Duration: 2 Months+ contract - Onsite Role Shift: 8 am – 4 pm/ No weekends/no on-call. Pay Rate: $57/HOUR. on W2 Note: Offering Best Pay. PER DIEM RATES PROVIDED TO THE CANDIDATE LIVING MORE THAN 50 MILES FROM THE FACILITY. Duties: Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process. Performs utilization management and quality screenings. In- Patient Case Management is required. Discharge planning exp required Requirements: 2 years of acute care experience strongly preferred Discharge planning, In-Patient Case Management is required BSN Preferred / ASN - Required.
