Hospice Registered Nurse (RN), Case ManagerFull-Time | Day Shift with On-Call RotationLocation: Cleveland, OhioPay: Starting at $35/hrProvide compassionate, patient-centered hospice care in the home and community setting. As a Hospice RN Case Manager, you will coordinate and deliver individualized care plans that prioritize comfort, dignity, and quality of life for patients and their families. This role offers meaningful, relationship-driven work within a collaborative interdisciplinary team committed to hospice philosophy and regulatory excellence.Benefits and Compensation Competitive hourly pay based on experience Full-time employment with flexible day shift schedule On-call rotation including evenings, weekends, and holidays as required Mileage reimbursement for home visits Comprehensive medical, dental, and vision coverage 401(k) with retirement planning support Life insurance, AD&D, and disability coverage Flexible spending accounts Employee assistance program Paid time off to support work-life balance Continuing education and professional development support Supportive team environment focused on mission and values QualificationsRequired: Graduate of a state-approved school of nursing Active, unencumbered Registered Nurse (RN) license in the State of Ohio Current Basic Life Support (BLS) certification Valid Ohio driver's license with acceptable driving record Ability to meet agency insurability standards Strong clinical assessment and care planning skills Ability to read, write, speak, and understand English Preferred: One year of nursing experience Prior hospice or home health experience CHPN certification Responsibilities Complete initial, comprehensive, and ongoing patient assessments Develop, implement, and revise individualized hospice plans of care in collaboration with patients and families Coordinate services and collaborate with the Hospice Team to determine scope and frequency of care Ensure compliance with Medicare Conditions of Participation and all federal, state, and local regulations Participate actively in Interdisciplinary Group (IDG) meetings Provide education to patients, families, caregivers, and staff regarding hospice philosophy, disease progression, symptom management, and end-of-life expectations Collaborate with physicians, facilities, and other healthcare providers regarding changes in patient condition Maintain accurate and timely documentation in accordance with regulatory and agency standards using electronic and paper records Provide supervision and oversight to hospice LPNs and nursing assistants Complete home visits and fulfill on-call responsibilities as scheduled Support a positive work environment aligned with organizational mission and values Engage in ongoing professional development and apply new knowledge to enhance patient and family care Health Advocates Network, Inc. is an equal opportunity employer.
