Together, let's drive excellence in healthcare. Summary Of Job Provides personal care and assistance to Home Care and Hospice clients under the supervision of the Case Manager and Clinical Manager. Recruitment Requirements High school diploma or equivalent preferred; Must be able to read and understand written instructions; Must be able to take direction via telephone in patient's home; Must be able to make meaningful observations and report changes in patient condition; Must have basic knowledge of computer applications and ability to enter patient information into an electronic device. Successfully pass all competency exams. Qualified for employment under current state (Pennsylvania or New Jersey) state regulations, current driver's license and access to a car, current car registration, and current car insurance.
