Join a compassionate team dedicated to transforming healthcare delivery through value-based care.Position Highlights As a Lead Physician, you will:Provide Exceptional Primary Care:Deliver comprehensive, patient-centered care to a panel of members.Coach & Mentor Providers:Guide clinicians on delivering primary, urgent, and community care while effectively leading interdisciplinary teams.Collaborate with Leadership:Work closely with the Medical Director to provide clinical leadership to physicians, nurses, behavioral health specialists, and medical assistants.Drive Market Goals:Play an integral role in achieving quality, utilization, and engagement goals by participating in leadership meetings and representing the organization to community and healthcare partners.Key Responsibilities Clinical Care: Deliverprimary care servicesto a panel of members in a collaborative, team-based environment.Manageacute exacerbations of chronic conditionsonsite or determine when a higher level of care is necessary.Facilitatesame-day/next-day urgent appointmentsfor members recently discharged from hospitals or skilled nursing facilities.Develop and implementevidence-based care plansin collaboration with patients, families, and care teams.Ensure patients receive robusttransitional carethroughout their continuum of care, including hospital discharges, emergency room visits, and home care.Leadership & Mentorship: Coach and mentorproviders and Advanced Practice Providers (APPs) to deliver exceptional care and achieve quality metrics.Facilitatepopulation health huddlesand multidisciplinary meetings to guide teams toward achieving clinical and utilization goals.Develop clinical protocolsto upskill clinical staff and ensure top-of-license practice.Administrative & Collaborative Duties: Partner with the Medical Director to fosterrelationships with academic programsandcommunity organizations.Track and achievespecified quality metricsto ensure optimal outcomes.Participate in leadership meetings and serve as anambassadorfor the organization to community-based organizations, hospital systems, and payer partners.Working Conditions Operates in aprofessional office environmentwith occasional exposure to clinical settings.May require wearingprotective attire, such as gloves and masks.Frequent travelto other centers or corporate offices as needed.Physical Requirements Ability to remainstationary for long periods of timewhile operating computers and office equipment.Mustcommunicate clearlyand exchange accurate information with patients, providers, and staff.RequirementsActive, unrestricted medical licensein the state of employment.5+ years of clinical experiencein primary care, community health, or hospital-based care.Experience leading interdisciplinary teamsin clinical or administrative settings.Strong expertise inchronic care management, value-based care models,and population health.Proven success inmentorship, team leadership,and healthcare service delivery improvement.Exceptionalproblem-solving, project management,andinterpersonal communication skills.BenefitsHealthcare United offers acomprehensive compensation and benefits packagetailored to support your leadership role:Compensation:Competitivebase salarywith performance prehensiveretirement and profit-sharing plans.Benefits:GenerousPTO and paid prehensivehealth, dental, and vision insurance.Malpractice coverage.CME allowanceandpaid CME time off.Relocation assistance andmonthly cell phone allowance.Professional Development:Opportunities forcareer advancement, mentoring,andleadership training.Support for clinicianmental health and well-being.Advance Your Career in Leadership and Care Innovation!
