Productivity Units: 30 units Responsibilities: Deliver skilled nursing care in patients' homes, following established care plans. Complete comprehensive assessments, including OASIS documentation. Administer medications, perform wound care, and provide patient/family education. Collaborate with interdisciplinary teams to coordinate patient care. Ensure timely and accurate documentation using Homecare Homebase (HCHB) . Manage admissions, recertifications, and discharges. Participate in on-call rotation as required. Qualifications: Required: Active RN license in North Carolina (or compact state). 1-2 years' experience as an RN in a clinical care setting. 2+ years of home health experience . Proficiency with HCHB software and OASIS documentation . Current CPR certification. Reliable transportation, valid driver's license, and insurance.
