American Traveler is seeking an experienced RN - Case Manager for a hospital-based Case Management position requiring a BSN, Epic proficiency, and a minimum of 3-5 years of acute care case management experience. Job Details • Work is performed in a hospital setting with 457 licensed beds, designated as a Trauma Level III, teaching hospital, Chest Pain Center, Stroke Center of Excellence, and Bariatric Center of Excellence, • Case management unit operates at approximately 99% capacity, with a daily patient caseload of 14-16 patients depending on unit assignment, • Patient population varies by unit and may include adult medical/surgical, cardiac, orthopedic, and pediatric patients, • EMR: Epic (required); Careport for referral management (preferred), • Communicates via Rover Chat in Epic and carries a work phone, • Schedule consists of five 8-hour day shifts per week with an 0800 start (0800-1630), • No weekend rotation and no on-call requirements, • Floating to any unit is possible, • Holiday coverage is required unless time off is approved in advance, Job Requirements • Active RN license in NC or a compact state currently in hand, • BSN required, • Current BLS certification required, • Case Management certification preferred, • Minimum 3-5 years of acute care case management experience required, with most recent experience within the last year, • Epic EMR experience required, • Proficiency in discharge planning including placement for SNF, IPR, LTAC, hospice, home health, DME, behavioral health, and DSS (peds), • Experience with complex and difficult placements, including working with homeless populations, • Must have traveled previously as an RN Case Manager - first-time travelers will not be considered, • Candidates must reside more than 50 miles from the facility to be eligible, • At least 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN, required for consideration, • Last 4 digits of SSN and full date of birth required for consideration, • Must be separated from any Advocate Health Southeast (AHSE) facility for at least 1 year following a permanent position before being eligible, Additional Information • Core responsibilities include patient assessment, care coordination, and discharge planning across a variety of acute care units, • Department is supported by a Director, Manager(s), CMO, a highly engaged UM team (including a PA), peer Case Managers, and Social Workers, • Departmental orientation ranges from 2-5 days depending on skillset and comfort level; a clinical interview with manager approval is required, • Department culture is described as highly supportive and teamwork oriented, with communicative leadership and a seasoned, knowledgeable staff, • RTO requires manager approval, • Scrubs: Navy blue required, • Free parking is available on site
