Job Description

We are seeking a dedicated Medical Social Worker with experience in acute care and case management to support inpatient populations in a hospital setting. This role focuses on care coordination, discharge planning, and psychosocial support for adult patients, including those with complex medical needs such as neurological and cardiac conditions. The ideal candidate is collaborative, adaptable, and experienced in interdisciplinary care environments. Key Responsibilities Conduct comprehensive biopsychosocial assessments for hospitalized patients Develop and implement individualized plans of care for patients and caregivers Provide care coordination and case management throughout hospitalization Lead and participate in discharge planning , including referrals to home care, rehabilitation, and long-term care facilities Coordinate services such as durable medical equipment (DME) , transportation, and community resources Facilitate care conferences with interdisciplinary teams Provide crisis intervention (e.g., mental health, substance use, safety risks) Educate patients and families on advance directives , community support programs, and financial resources Assist with access to programs such as Medicaid, Medicare, housing support, and public assistance Maintain accurate and timely documentation in the electronic medical record system Collaborate with healthcare professionals including nursing, therapy services, pharmacy, and social services Qualifications Master’s Degree in Social Work (MSW) required Active BLS certification and LSWAIC or LCSW (or equivalent) required Minimum 1 year of inpatient/acute care hospital experience, including case management and discharge planning Experience working with high-risk patient populations and providing crisis intervention Proficiency with Epic or similar electronic medical record systems Strong knowledge of community resources and social support services Ability to manage high patient volumes effectively Excellent communication, teamwork, and interdisciplinary collaboration skills Demonstrated cultural competency and commitment to patient-centered care Important Details: 13-week contract position (extension possibly) Shift is 8 hrs/day shift (8 AM - 4:30 PM), 40 hrs/week Location is Seattle, WA 98122 Pay Rate for Locals: $42 - $46 hourly Call to Action: Spots Are Filling Quickly — Apply Now!
