The Case Manager predicts, facilitates and evaluates the transition of patients across the care continuum and links people to systems that provide them with resources, services and opportunities to achieve and maintain maximum functioning. Requirements Education: Associates Degree in Nursing with ongoing efforts to obtain Bachelors Degree. Experience: Minimum of 3 years clinical experience required.
