Key Highlights - Weekly pay range: $2,452–$2,577 per week - Contract duration: 13 weeks - Shift: Day shift, Monday–Friday, 8:00 a.m.–4:30 p.m. - Location: Portland, Oregon, US - Position type: Travel - temporary assignment - Setting focus: Home care coordination and patient management Responsibilities - Plan, organize, and direct home care services to meet patient needs in the home. - Manage a patient caseload in an efficient and effective manner. - Provide patient care within the scope of practice, while collaborating with core disciplines to meet the patient’s care plan and goals. - Serve as a primary resource for patients, family members, and other care team members; communicate care needs and progress. - Assess patient conditions, implement and monitor care plans, and adjust as needed to achieve care goals. - Document and coordinate activities in alignment with established care plans and documentation standards. Requirements - Active Registered Nurse license - BLS (Basic Life Support) certification - Valid Driver’s License and current auto insurance - Must be at least 21 years of age - Ability to travel to patient homes and work in a home care coordination role Please note: Pay rates and guaranteed hours are estimates and may vary.
