Home Health Physical Therapist Job Type: Full-Time/Part-Time (In-Person) Practice Setting: Home Health - Patient Homes Schedule: Flexible, Visit-Based Day Schedule Patient visits are conducted in homes throughout Northern Virginia , Primarily including: Fairfax County, Falls Church, Annandale, Fredericksburg, Spotsylvania & Stafford Job Summary: We are seeking a compassionate and dependable Physical Therapist to provide skilled home health therapy services to patients throughout Northern Virginia , primarily in Fairfax County, Falls Church, Annandale, Fredericksburg, Spotsylvania, and Stafford . The Physical Therapist will focus on restoring mobility, improving functional independence, and supporting safe recovery while delivering care in patients' homes. All services must be documented accurately in accordance with agency policies, Medicare/Medicaid regulations, and applicable clinical standards. Key Responsibilities: Conduct initial, follow-up, and discharge assessments in patients' homes. Develop and implement individualized physical therapy plans of care. Provide therapeutic exercises, gait training, balance training, and mobility interventions. Educate patients and caregivers on safety practices, fall prevention, and home exercise programs. Monitor patient progress and modify treatment plans as clinically appropriate. Collaborate with physicians, nurses, and interdisciplinary healthcare team members. Document all patient visits accurately and in a timely manner within the Electronic Health Record (EHR). Participate in case conferences and interdisciplinary care coordination meetings. Ensure compliance with Medicare, Medicaid, and regulatory guidelines. Maintain professional, ethical, and clinical standards of practice. Qualifications: Active Virginia Physical Therapist License . CPR Certification . TB Screening . Reliable transportation for home health visits. Knowledge of Electronic Health Records (EHR) . Strong patient assessment and treatment planning skills. Familiarity with Medicare guidelines and regulatory compliance . Home health experience preferred . OASIS documentation experience strongly preferred . Experience: Preferred: Minimum 1 year of clinical experience . Strong new graduates with relevant clinical rotations will also be considered. Compensation & Benefits: Compensation: Approximately $70 - $90 per visit (Approx. $35 - $55 per hour ). Flexible scheduling. Mileage reimbursement. Referral program.
