Internal Medicine Nurse Practitioner

Nurse Practitioner Comprehensive Adult Care We are seeking a compassionate, mission-driven Nurse Practitioner to provide high-quality, value-based care to older adults with complex medical needs. This role emphasizes outcomes over volume and close collaboration with an interdisciplinary care team. Responsibilities Perform comprehensive adult and geriatric assessments in clinic, home, senior living, and via telehealth Manage transitions of care to reduce avoidable hospital and emergency department visits Identify care gaps and collaborate with primary care providers to optimize care plans Evaluate and manage non-life-threatening acute conditions Support chronic disease management, preventive care, and advance care planning Schedule Full-time, MondayFriday (8am5pm) Qualifications Current Oregon Nurse Practitioner license ANCC or AANP certification 35 years of NP experience (Primary Care, Geriatrics, Internal Medicine, Family Medicine, or Palliative Care preferred) DEA licensure (or ability to obtain) and CPR/BLS certification Spanish bilingual a plus Compensation & Benefits $Up to $150K salary $10,000 sign-on bonus Medical, dental, vision, 401(k), paid time off, and professional development support We encourage candidates from all backgrounds to apply, even if you dont meet every qualification.
