Our mission at MMG is to improve lives through compassionate quality healthcare and be the trusted choice for health and wellness.JOB RESPONSIBILIES • This position will act as a liaison to the Medication Therapy Management (MTM) program but will also work on transition care and other integrated care delivery needs.• Works in collaboration with the MTM program on outpatient medication management and utilization for MHS employees and families• Works closely with the ACO to help manage and develop preventative care, quality improvement and chronic disease management programs• Works with Hospitalist program and Primary Care physicians to develop, implement and manage transition of care management program• Reports directly to CMOQUALIFICATIONS • Board-certified Nurse Practitioner • Active Texas medical license• Minimum 2 years of experience in clinical practice• Strong interest in Medication Therapy Management• Ability to work effectively in a fast-paced, team-oriented environment
