We are seeking a full-time Family Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join a busy FM clinic with South Texas Health System Clinics in Weslaco, Texas. Quick Facts • Employment Type: Full-Time • Schedule / Hours: Standard clinic hours with on-call rotation for after-hours and weekends • Call: Rotating call coverage to support physicians • Patient Volume: Diverse primary care patient population including Medicare Advantage and Medicaid patients • Location: Weslaco, Texas (Rio Grande Valley) • Practice Setting: Outpatient primary care within a multispecialty integrated health system • Support / Care Team: Collaborative team including physicians, APPs, and managed care resources • EMR / Technology: Electronic medical record system Responsibilities • Provide comprehensive family medicine care in person and via telemedicine • Support physicians with value-based care initiatives and HEDIS measure completion • Manage patient transitions from emergency room and inpatient discharge to outpatient follow-up within 7 days • Educate patients on proper utilization of emergency and urgent care services • Collaborate with physicians to ensure accurate coding, risk adjustment, and quality documentation • Participate in after-hours and weekend call rotation • Cover for physicians during leave periods • Champion coordinated care models for Medicare and Medicaid populations Compensation & Benefits • Salary: Competitive compensation package • Sign-on / Relocation: Available upon qualification • Health, Dental, Vision & Retirement benefits including 401(k) • PTO, CME allowance & Schedule Flexibility • Wellness & Work-Life Balance support Qualifications • Licensed Nurse Practitioner or Physician Assistant with Board Certification in Family Medicine • Active or eligible Texas license • Prior family medicine experience preferred • Spanish fluency strongly preferred Growth & Development • Opportunities for professional development, mentorship, and leadership roles within the health system • Engagement in quality improvement and managed care initiatives About the Organization & Community South Texas Health System Clinics is part of a comprehensive regional healthcare system serving the Rio Grande Valley, with six hospitals, freestanding emergency rooms, and a broad specialty referral network.
