Their services include home-based primary care, palliative care, hospice care, chronic illness management, home health, counseling services, and more.POSITION DESCRIPTION: The Nurse Practitioner or Physician Assistant is responsible for delivering comprehensive primary care to a dedicated panel of geriatric patients. This includes visiting assisted and independent living facilities (ALFs / ILFs), private homes (house calls), and other group settings (apartment buildings, senior housing complexes, etc.).The duties of the role include, but are not limited to:- conduct comprehensive physical examinations- diagnose and treat illnesses- manage chronic conditions- engage in advanced care planning discussions- coordinate care with other healthcare providers- provide palliative care consultationsProviders see an average of six (6) patients per day.
