Job Title: Inpatient Social Worker (LSW/LCSW) – Travel Contract Location - Fort Wayne, Indiana Shift: 8-Hour Day Shift (Monday–Friday, 8:00 AM – 4:30 PM) Contract Length: 13 Weeks Guaranteed Hours: 40 Hours per Week Work Setting: Inpatient Rehabilitation (On-site) Pay ( traveler ) - $60 - $64 Pay ( Local Candidate) - $50 - $52 Job Overview We are seeking an experienced Inpatient Social Worker (LSW/LCSW) for a travel contract opportunity in an inpatient rehabilitation setting. The Social Worker will collaborate with the interdisciplinary care team to support patient assessments, discharge planning, and care coordination to ensure smooth transitions to the next level of care. This role requires strong clinical assessment skills, the ability to work independently, and effective collaboration with healthcare professionals to support patient recovery and discharge planning. Key Responsibilities Complete comprehensive patient assessments to evaluate patient status and support discharge planning. Participate in weekly interdisciplinary care team conferences. Coordinate with the rehabilitation care team to drive patient stay progression and facilitate timely discharges. Provide patients and families with resources and support for post-discharge care. Develop and implement effective discharge plans to ensure smooth transition to the next level of care. Maintain accurate and timely documentation in Epic. Collaborate with physicians, nurses, therapists, and other healthcare professionals. Required Qualifications Current Indiana LSW or LCSW License (Required) Minimum 2 years of experience as an Inpatient Social Worker (IP LSW) Inpatient rehabilitation experience strongly preferred Strong ability to work independently and collaboratively within an interdisciplinary team Experience with Epic electronic medical record system preferred Schedule & Work Environment Shift: Monday – Friday, 8:00 AM – 4:30 PM Lunch Break: 30 minutes No call required No floating to other departments On-site position Unit Details Department: Inpatient Rehabilitation Bed Capacity: 50 beds (expanding to 62 beds pending approval) Collaborative care team environment focused on rehabilitation and patient recovery.
