Job Description & Requirements Specialty: Family Practice Discipline: Nurse Practitioner Start Date: 04/06/2026 Duration: 52 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Position: Primary Care Nurse Practitioner Location: Pittsburgh, PA 15206 Job type: Permanent, Full-time Pay Rate: $60/Hour Job Description: We are seeking a full-time Primary Care Nurse Practitioner (NP) to deliver high-quality medical services to patients residing in long-term care and skilled nursing facilities. This role is designed for a compassionate clinician with experience managing chronic conditions and acute changes in elderly populations. You will work collaboratively with an interdisciplinary care team, ensuring continuity of care and quality outcomes. Responsibilities: Provide primary and preventative care to long-term care residents. Assess and manage acute and chronic medical conditions. Conduct advanced care planning and chronic care management. Prescribe appropriate medications, including controlled substances per PA law. Order, review, and follow up on diagnostic testing and treatments. Collaborate with facility staff and physicians to optimize patient care. Maintain accurate EMR documentation and participate in care team communication. Requirements: Current, unrestricted Nurse Practitioner license in Pennsylvania. Board Certification (Adult, Family, or Geriatrics) . DEA licensure (required to prescribe controlled substances). Master's degree in Adult-Gerontology NP specialization. Bachelor's degree in Nursing (RN to BSN). Clinical experience in adult, family, or geriatric care. LTC experience preferred. Proficiency in using EMR systems. Experience managing patients with advanced illnesses and multiple chronic conditions.
