Registered Nurse - Case Management- Case Manager

Job Details Transitions Program DepartmentOrientation will be approx 2 weeks in office at Kaiser Martinez 200 Muir Rd MTZ 94553.After orientation the job will be remote--Applicant must have in home Wifi. A laptop and phone will be provided. Candidates must reside within 60 miles of the facility for the duration of the assignment. Transitions is a short term, telephonic, case management program. Patients are managed x 30 days with a minimum of weekly phone calls. The caseloads are 25-35+ patients. We are open 7 days a week from 8:00a to 4:30p.
