Travel RN Job Details Job Title: RN Transplant Coordinator / Post-Lung Transplant Nurse Coordinator Location: Boston, MA 02114 Assignment Type: Travel contract (travel nursing) Start Date: 03/22/2026 End Date: 06/20/2026 Duration: 13 weeks Schedule: 40 hours/week Shift: 8:00 AM - 4:30 PM Shift Duration: 8-hour days Setting: Outpatient Transplant Surgery (Post-Lung Transplant Clinic) Weekly Estimated Pay: $2,750-$2,890/week RN Transplant Coordinator Requirements License: Active Registered Nurse (RN) license required Experience: 3 years ICU RN experience required Core Focus: Post-transplant care coordination, patient/family education, outpatient follow-up support, and collaboration with a multidisciplinary transplant team Key Responsibilities (Post-Lung Transplant Nursing) Serve as the central point of contact for post-lung transplant recipients and families during the post-discharge phase Provide outpatient nursing care to support optimal physical, social, and emotional rehabilitation after lung transplantation Assess and address educational, psychosocial, and economic needs; reinforce post-transplant education and follow-up requirements Monitor and support patient compliance with medications and treatment regimens; collaborate with the transplant team to improve adherence Collect, review, and document clinical data related to recipient health and organ function in the patient chart/medical record Coordinate labs and diagnostic testing for clinic visits; communicate results to transplant providers and multidisciplinary care team Implement medication and treatment changes as ordered by credentialed providers Act as a resource on post-transplant protocols for internal and external providers (e.g., testing schedules, high-risk labs, outpatient rejection treatment) Arrange consultations, procedures, infusions, and hospitalizations when indicated (e.g., direct admissions, ED transfers, IV infusions, bronchoscopies, sleep studies, thoracenteses) Maintain communication with referring physicians, primary care providers, and other members of the care team Maintain thorough, timely, and accurate documentation across all patient interactions and care coordination activities Apply now to join a leading transplant program and advance your travel nursing career in Massachusetts healthcare.
