Speech Language Pathologist - SLP

Required skills: Dysphasia Management, Patient Safety, Aural Rehabilitation, Functional Maintenance Programs, In-service Training, Patient/Family Education, Evaluation Tools (BIMS, Depression Screening, Oral Peripheral Exam), Billing/Documentation Must have home health experience; OASIS training preferred Certs required: BLS, CCC-SLP, state licensed Scrubs: Any solid color scrub Patients: Acute/chronic adolescent through geriatric Skilled Home Care (Medicare, Medicare Advantage and Commercial ins.) Home skilled visits - 6-8 per day (MINIMUM of 6 points per day at managers discretion may increase by 1 point) 1-2 Weekends per contract Laptop, MiFi for internet access, Portable printers BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system Other requirements: 1 year experience required Must have valid drivers license and car to make home visits.
