About the Role: In the role of RN Case Manager, you will play a vital role in guiding patients and their families throughout their healthcare journey. You'll be responsible for coordinating services, connecting vital resources, and creating individualized care plans tailored to each patient's unique requirements. Collaborating with physicians, nurses, and interdisciplinary leaders, your aim will be to ensure smooth transitions and maintain clear communication for a high-quality care experience. Your Key Responsibilities Will Include: Assisting patients and families with program orientation, including explaining rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights. Coordinating educational sessions for patients and caregivers to promote active participation in treatment planning and goal-setting discussions. Designing and implementing personalized treatment plans that address the patient's individual strengths and personal recovery goals. Conducting psychosocial assessments and discharge planning evaluations, accurately documenting findings and sharing insights with the care team. Working closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to deliver seamless, patient-centered care. Qualifications Required: Graduate of an accredited nursing program-preferred candidates will hold a Bachelor of Science in Nursing. Active RN license in Texas or a Multi-State RN Compact License (Texas RN License required within 90 days of hire for Compact License holders). Minimum of 3 years of RN experience. At least one year of experience in utilization review, resource management, discharge planning, or case management.
