Job Description & Requirements Specialty: Speech Language Pathologist Discipline: Therapy Start Date: 04/27/2026 Duration: 12 weeks 40 hours per week Shift: 8 hours Employment Type: Travel Description ========= Job Responsibilities: --Performs Patient assessments: -Accurately assesses patient’s appropriateness for home care, learning needs and progress toward goals. -Collaborates with patient, family, and physician to develop the plan of care for the patient. -Performs baseline assessment of patient swallowing, communication, and cognitive abilities as indicated. --Maintains safe environment for patients and co-workers: -Assesses social environment and adjust or provide education as needed. -Assess for potential clinical risks and adapt program as indicated. -Follows universal infection control measures. --Implements treatment program to achieve designated goals: -Administer occupational therapy services per physician orders. -Instruct in communication and safe swallow, cognitive training as indicated. -Develop and instruct in therapeutic exercise program. -Implement program to restore function, and achieve maximum functional status. -Provides discharge planning and referral to resources as needed. -Communicate with physician, nurse, and other clinical disciplines as necessary. -Develops/implements/evaluates plan of care based on evidence-based knowledge. -Revise care plan as indicated within physician orders. --Uses age appropriate teaching methods for patients and families: -Evaluates teaching plan and revises as necessary. -Documents patient education tools utilized. --Demonstrates compliance with agency policies: -Assumes responsibility for Case Management. -Demonstrates knowledge of home care regulations and insurance reimbursement guidelines. -Assures compliance with regulatory requirements. -Follows agency documentation policies. -Shows evidence of interdisciplinary communication. -Ensures appropriate utilization of services. -Follows scheduling process. -Maintains visit productivity standards. -Participates in Team and IDT meetings. -Prioritizes schedule to accommodate patient's needs. -Travels to patient homes and facilities as indicated. -Complies with paid time off request process and weekend/holiday requirements. --Assists with growing agency business: -Provides orientation to new employees. -Accepts other assignments as requested. -Cross trains to learn new skills. -Use of personal vehicle is required for home visits. Skills ==== Required Skills & Experience: -Minimum of one (1) year of experience in acute, subacute or home care setting. -Good customer service and communication skills. Preferred Skills & Experience: -N/A Education ======== Required Education: -Graduate of an accredited speech and language program with a minimum of a Master’s degree.
