Our focus is to promote healthy lifestyles, protect and nurture vulnerable children and youth and empower them to have a voice in their communities.SUMMARY:The Adoption Case Manager (ACM) will be responsible for assuring compliance with Adoption State and County regulations for children, youth and families for whom APM has received a Statewide Adoption and Permanency Network (SWAN) referral.
