Social Service Consultants work directly with patients-both in person and by phone-to assess social needs, connect patients to critical community resources, and reduce barriers to care.This position is grant-funded for an anticipated three-year period, pending final grant approval with continued employment contingent upon ongoing funding availability and organizational needs.SSCs collaborate closely with medical providers, behavioral health clinicians, and community health staff to deliver comprehensive, patient-centered care addressing social determinants of health in a culturally responsive and holistic manner.Hourly Rate:For candidates with their bachelor's and higher: 19.50-22.50/hourFor candidates with their HS Diploma: 17.00-20.00/hourKey ResponsibilitiesMeet with patients in person or by phone to assess social determinants of health and provide culturally appropriate resourcesMaintain accurate documentation in the Electronic Medical Record (EMR)Provide limited case management services for an assigned panel of patients living with HIVProvide crisis intervention and referrals for patients experiencing domestic violence or child abuseConduct HIV counseling and testing in accordance with established guidelinesAssist uninsured patients with enrollment in health insurance programs and follow up to address barriersSupport patients with insurance renewal processes as neededFacilitate referrals to Early Intervention and Autism Evaluation servicesWork with children and their families to support linkage to needed resources and assist with navigating the educational system, welfare system, and other external agencies impacting health and well-beingWhat We OfferFixed Monday-Friday schedule (no evenings, weekends, or on-call requirements)Meaningful, mission-driven work serving underserved communitiesOpportunities for professional growth and skill developmentThree one-day, organization-wide staff retreats each yearCompetitive paid time off including vacation, sick, and holidaysOne-month paid sabbatical after seven years of serviceMedical and dental insurance, retirement plan, and short- and long-term disability coverageTuition reimbursement program after one year of employmentEligibility for federal and state loan forgiveness programsQualifications & SkillsBachelor's degree preferredBilingual in English and Spanish requiredOtherwise, qualified candidates who are not proficient in Spanish may be eligible for up to 12 weeks of language immersion trainingAbility to work effectively in a fast-paced, team-based environmentStrong communication, organization, and interpersonal skillsPassion for innovative approaches to delivering high-quality social servicesCommitment to working in a Christian healthcare ministry grounded in compassion, dignity, and serviceEqual Employment OpportunityEsperanza Health Center is an equal employment organization.
