Come join us!ABOUT THE TEAM The Supported Living department provides both at-home and community supports for people with intellectual and developmental disabilities during morning, mid-day, and evening hours. We focus on each person's individual goal, gifts, and areas of interest as we customize schedules to support people within their communities every day. This Coordinator will work alongside adults with intellectual and developmental disabilities and their team as they prepare and eventually move into a new home.IN THIS ROLE, YOU'LL: Support a person through a rigorous person-centered planning process to determine their personal goals and desires related to moving into a new home in a new community.Ensure a smooth transition for all new people entering the Supported Living Program to include, as needed: finding and setting up housing arrangements based on their wants and needs, facilitating searching for and interviewing potential roommates for the individual, etc.Coordinate services from Social Security Administration, Service Coordination and Social Services for people.Complete necessary paperwork to maintain Medicaid & social security benefits & other related forms in collaboration with SEEC's benefit specialist.Advocate for the person's right to be live, work, and thrive as they choose. This involves learning about rights and responsibilities, supporting independent decision-making and problem solving, as well as modeling and practicing conflict resolution.Help individuals live holisticallyEnsures that each person is receiving the appropriate services as their individual plan dictates and billed accordingly.ABOUT YOU Bachelor's degree in a related field; With 5 years' experience in related field and two years of supervisory experience, preferably within a residential or home setting.Associates and 5-7 years' experience in a related field and four years of supervisory experience.Housing Support Services Certification requiredMust be flexible to work in the Mornings and/or evenings during the weekday and weekend as needed.Must be sincerely committed to supporting people with disabilities to become active members of their communities, along with the well-being and professional development of direct reports.Must be willing to travel throughout the Maryland and District of Columbia (DC) region.Case management Experience.Experience in local, state and federal housing voucher programs (Preferred)Developmental Disabilities Administration (DDA) services experience (Preferred)WE OFFER Health, Dental & Vision Plans.Competitive PayLTD & Basic Life InsurancesMatching 403BPaid Vacation.Tuition Assistance Program.Employee Program Assistance.And more!SEEC is an equal opportunity employer committed to hiring a diverse workforce in an inclusive culture environment.
