Assure service planning and delivery are done through a person-centered approach. Assist in monitoring the delivery of services and provider compliance with quality assurance standards. Complete other functions to support agency operations.Essential Functions/ResponsibilitiesThe following duties are normal for this classification. Other duties may be required and assigned.Position Duties Coordination and Evaluation of ServicesDevelopment and Review of MnCHOICES Support PlanAdministrative Case Management ActivitiesProvide Rule 185 Developmental Disabilities Case Management ServicesNon-Essential FunctionsPerform job related duties as assigned or apparent.Knowledge, Skills, AbilitiesAbility to make case management decisions.Knowledge of physical and mental disabilities and their effects on individual functioning.Knowledge of family dynamics; community resourcesKnowledge of social and economic problemsKnowledge of and ability to adhere to social work methods, values, and principles.Knowledge of philosophy and trends in mental illness and mental health issues.Effective interviewing assessment, counseling, evaluation, and service coordination.Ability to foster and maintain effective working relationships.Ability to develop and coordinate resources.Ability to utilize the MMIS and SSIS systems required.Knowledge of and ability to comply with data privacy laws and compliance.Knowledge of the principles and techniques of interviewing and documenting.Knowledge of individual and group behavior.Knowledge of social casework objectives, principles, and methods.Knowledge of social-economic factors.Knowledge of family systems and dynamics.Working ability to work constructively within an agency in the community setting and in effectively utilizing appropriate resources and services.Ability to work constructively in the development and coordination of community resources.Ability to manage time productively.Ability to prioritize caseloads.Ability to communicate effectively, both orally and in writing.Minimum QualificationsEducation and ExperienceA bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field orA bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency orA public health nurse orA registered nurseRequired Training During OrientationTraining on Person Centered Thinking and PlanningMandated Reporter TrainingAnnual Data Privacy, Security, Fraud and OSHA TrainingSSIS and MMIS TrainingOther trainings as related to positionRequired Certifications/LicensesValid driver's license and ability to maintain such license and proof of insurance.Preferred Certifications/SpecialtiesSocial Worker Licensure preferredQualified Developmental Disability Professional (QDDP)RelationshipsIndividuals needing long term services and support and their families and/or legal guardians.Regular interaction with other County Social Workers, Public Health and Attorney office staff.Tier 2 and Tier 3 service providers, medical, psychiatric, psychological, legal, educational, vocational, rehabilitation and other professionals and resources.Department of Human Services staff.Supervisory ResponsibilitiesNo direction supervision of employees.Working ConditionsWork is mainly performed in an office setting but may also be in clients' homes, residential facilities, and other county offices.Frequent travel may involve exposure to inclement weather conditions, unsanitary conditions in homes and unsafe environments.Direct service provision may require dealing with persons who are disoriented, hostile, aggressive, abusive or threatening.There may be hours worked outside of the normal business hours.Ability to work from homePhysical Requirements Of This PositionDesk work involving use of a computer, telephone, and other devices.Communicate effectively to contacts listed under Internal/External Relationships above.Ability to read screens and documents.Ability to drive in various light and weather conditions.Ability to type and use a computer or mobile device.Office work may include reaching for documents, grasping documents, standing, walking, and repetitive motions.Lift and carry files and material weighing up to 25 pounds.
