Home Health Case Manager

Job SummaryAssess, plans, implements and provides direct nursing care. The case manager is responsible for facilitating the patient's plan of care from admission through discharge. The RN case manager coordinates the patient care with providers, nurses, therapists, social worker, aids, outsides (insurance's, DME's, pharmacies, APS, AAA, Meals for the Elderly, Specialized Transportation, etc.) and the patient's family and/or caregivers as needed in an efficient and cost effective.
