Position Summary: Provide education and support to clients in recovery for a mental health disorder. Participate in outreach to community partners to help coordinate services for clients in order to support their recovery. Maintain regular contact with clients to monitor and assess progress. Position Description:Assist staff with caseloads through regular client contact in community and program setting. Track and assess progress as determined by program guidelinesComplete reports as request by our partnersAssist with the reduction of recidivism when working with hospitals, halfway housing and family care homes.Help to develop and implement Individual Treatment Plans Efficiently operate personal computer to enter, edit and/or retrieve data using agency electronic health record Assist clients in accessing community resources, including, but not limited to food, clothing, supportive housing, Medicaid, job readiness and/or placement, public assistance, etc. Facilitate site-based daily support/peer groups Visit, support, and transport clients as needed upon discharge from program Collaborate with family members, internal and external case managers, and other providers while supporting client's stability and recovery Prepare documentation in compliance with contract, regulatory and Agency requirements Other duties as assigned Required Knowledge, Skills and Abilities:Knowledge of mental health disorders and community resources to assist with maintaining stabilityEffective verbal and interpersonal communication skills Ability to advocate and support clients involved in the treatment providing systemsAbility to effectively communication and present to partners clients progress, barriers and motivationsAbility to operate personal computer to enter, edit and/or retrieve data and to produce reports and correspondence as necessaryHigh level problem-solving skills and ability to multi-task while prioritizing tasks Job Requirements:High School Diploma or equivalent Lived experience with Mental Health DisordersExperience working with clients with history of mental illness. Peer Support Specialist certification or ability to obtain within first three months of employment Valid driver's license and current auto insurance may be required Essential Functions/Physical Demands:Positional: Driving in accordance with job duties assigned. Infrequent standing and walking. Constant sitting.Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching, handling and fingering.Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking and listening.
