As our Medical Records Documentation Audit Coordinator, you play a mission critical role ensuring that every participant's record is accurate, complete, and compliant - supporting exceptional care delivery across the entire program. If you enjoy auditing, quality assurance, regulatory work, and improving documentation workflows, this role offers meaningful impact, cross department collaboration, and the opportunity to shape high quality care for a vulnerable population. **What You'll Do Quality & Documentation Oversight** - Perform routine and targeted audits of participant medical records - Identify documentation gaps and coordinate corrective actions - Track trends and prepare reports for leadership, QAPI, and regulatory needs PNDS & Plan of Care Management - Maintain PNDS requirements and monitor system wide compliance - Audit Plan of Care documentation for completeness and regulatory alignment - Work closely with clinical leaders to address deficiencies Epic Workflow Support - Manage closure of hospital admissions in Epic - Reconcile discharge documentation and ensure timely completion - Partner with the RN Educator on documentation training and workflow improvement Health Information & Cross Functional Support - Support HIM reviews and medical record reconciliation - Serve as a liaison between clinical teams, administrative operations, HIM, training, and finance - Assist with survey readiness and internal compliance reviews **Requirements:** - Associate's degree - Minimum of 2-3 years in a healthcare role, quality assurance, compliance, or HIM-related experience. - Experience supporting audits and documentation reviews. - Strong organizational, analytical, and communication skills. - Proficiency in Microsoft Office applications, including Excel. **Preferred** : + Bachelor's degree + Experience working with electronic medical record systems (e.g., Epic, TruChart, or similar). - Background in clinical operations, rehabilitation services, or interdisciplinary healthcare settings. **Why Eddy SeniorCare PACE?** - Mission driven work supporting older adults - Collaborative, interdisciplinary environment - Opportunities to influence quality and compliance processes Ready to help elevate documentation quality and support exceptional participant care? Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
