**No Block Schedules, no exceptions****5 Days or Less of RTO******WA state license required in hand****Start Date: ASAPShift: 5x8 Days 0800-1630, alternating weekendsQualifications:"Master s Degree in Social Work (MSW) from an accredited institution."Licensed in Washington State (LICSW/LACSW)."Minimum 2 years of experience"Strong understanding of trauma-informed care, safety planning, and community resources."Familiarity with CMS regulations and documentation standards."Excellent communication, organizational, and crisis intervention skills."Ability to work collaboratively in a fast-paced, interdisciplinary environment.Key Responsibilities:Clinical Rounding & Patient Engagement"Attend daily interdisciplinary rounds; actively contribute to care planning."Maintain up-to-date knowledge of each patient s psychosocial status and discharge plan."Serve as liaison between patients, families/support systems, and outpatient providers.Chart Review & Risk Identification"Review mental health history and precipitating events leading to hospitalization."Identify and respond to chart flags including:oMental Health Advance DirectivesoFirearm access concerns (e.g., Donna s Law, Safe Storage, ERPO)oRequests for family/provider contact (verify ROIs prior to outreach)"Provide education on safety planning, firearm safety, and family support resources.Psychosocial Assessment & Care Coordination"Complete psychosocial assessments within 72 hours of admission."Identify current providers, scheduled appointments, and transportation needs."Document using standardized SmartPhrase (.BHUSWA).Discharge Planning & Appointments"Ensure CMS-compliant follow-up with medication providers within 30 days of discharge."Coordinate with medical team to ensure adequate medication supply at discharge."Update After Visit Summary (AVS) and document appointment details using SmartPhrase (.BHU30DAPP).Referrals & Community Resources"Facilitate referrals to appropriate outpatient services including:oPartial Hospitalization ProgramsoPathlight and culturally appropriate counselingoOther specialized programs based on patient needs"Ensure signed ROIs are in place for all referrals."Provide patients with tailored resource lists and AVS updates.Mandated Reporting & Safety"Independently report concerns to Adult Protective Services (Client) or Child Protective Services (CPS) as required."Educate staff on mandated reporting responsibilities; social work does not report on behalf of others.Resource Management & Logistics"Assist patients with transportation (Lyft, bus passes) and other discharge logistics."Collaborate with BHU leadership to manage access to transportation tools and SmartPhrase documentation.Team Communication & Handoff"Utilize FYI documentation and *** for handoffs and updates."Participate in team discussions to optimize discharge planning and continuity of care.________________________________________Qualifications:"Master s Degree in Social Work (MSW) from an accredited institution."Licensed in Washington State (LSWAIC, LICSW preferred)."Strong understanding of trauma-informed care, safety planning, and community resources."Familiarity with CMS regulations and documentation standards."Excellent communication, organizational, and crisis intervention skills."Ability to work collaboratively in a fast-paced, interdisciplinary environment.
