**Employment Type:** Part time **Shift:** Day Shift **Description:** **Position Purpose** The Referral Support Specialist (RSS) plays a key role within the Trinity Health At Home (THAH) Customer Care Center (C3). Serving as an on‑site partner to hospital-based liaisons (HCCs), the RSS ensures seamless referral entry, accuracy of documentation, and insurance verification for designated payers. This position supports high-quality patient transitions by managing referrals from initial receipt through completion within Homecare Homebase (HCHB) and related systems. The RSS helps ensure patients receive timely care while upholding Trinity Health's values and standards of excellence. **What You Will Do** + Review and screen incoming referrals to determine appropriateness of care based on THAH guidelines. + Accept, import, and enter referrals into HCHB and maintain complete and accurate documentation in the electronic medical record. + Verify insurance authorization for applicable payers and validate referral accuracy using established checklists and resources. + Serve as the central liaison between the referral source, families, and THAH agencies to support a smooth referral process. + Ensure timely transmission of referral information to appropriate agency contacts for efficient scheduling. + Meet productivity standards and take ownership of follow‑up tasks to ensure patients are seen within required timeframes. + Enter Remote Patient Monitoring (RPM) referrals into EPIC accurately and promptly. + Communicate effectively with the Virtual Care Center/Intake regarding barriers or process updates. + Navigate multiple applications and online systems while maintaining professionalism, confidentiality, and adherence to Trinity Health's Integrity Program and Code of Ethics. + Support pre‑active and lost kit follow-up work as needed. + Perform additional responsibilities as assigned based on business needs. **Minimum Qualifications** + High School diploma required; Associate's degree preferred. + 2-4 years of industry-related experience. + Working knowledge of governmental reimbursement criteria; familiarity with third‑party/managed care contracts preferred. + Proficiency in Microsoft applications; experience with THAH software platforms preferred. + Previous healthcare experience demonstrating strong computer literacy. + Commitment to Trinity Health's mission, ethics, and Behaviors of Excellence. **About Trinity Health At Home:** Trinity Health At Home is a leading provider of home-based care, rooted in a mission to serve with compassion and excellence.
