Summary: + The Social Worker Team Leader will primarily support Social Work activities; manage the performance of Social Work staff and provide them with continuing educational programs/training and collaborate with the Social Work Manager to develop efficiency within the department. Team Leader will address psychological, social and system issues of complex patients; communicate with insurers, interdisciplinary care team and Physician Advisor and lead discharge planning activities to assure timely coordination of home care, home medical equipment, placements, hospice, community resources, transportation. Other duties will include participating in quality data collection and risk management referrals. The expected weekly caseload of the Social Work team leader is adjusted to ensure achievement of team leader accountabilities. Responsibilities: + Staff Resource and Mentor + Maintains availability on patient care units to provide support to Social Work staff + Provides frequent coaching, mentoring and constructive feedback to ensure achievement of targeted performance levels + Serves as a resource for staff regarding inquiries or problems with Canopy functionality + Coordinate and facilitate supervision groups, team meetings, projects and initiatives with Social Work Staff + Department Liaison for Payer Inquiries + Acts as the first escalation point for Social Work staff, before escalation to Physician Advisor and/or Director for patient accounts where lack of insurance is creating placement issues + Elevates cases to the Physician Advisor and/or Director when payer complications still exist after the team leader has performed due diligence to resolve the case. + Performance Management: Quality Reviews + Complete quality reviews for Social Work staff to ensure appropriate performance and documentation + Provides constructive feedback of quality reviews, including an explanation of the score provided, areas for improvement and comments about positive performance + Communicates significant changes in individual quality review scores to the Social Work Associate Director + Performance Management: Report Analysis + Review reports available through AllScripts/Canopy/ECIN focusing on metrics that can be used to provide feedback to Social Work staff + Provide weekly or biweekly update to Social Work Associate Director on significant reporting trends to target significant training opportunities and/or individual performance concerns. + Educational Coordinator + Contributes to the design and development of professional standards of practice, hospital and department goals, workflow design, and department operations and documentation systems + Implements processes for collecting customer service/patient satisfaction comments and disseminating feedback to the Clinical Resource Management and Social Work Department + Leads development and implementation of department's annual educational programs + Assures thorough and detailed orientation is developed for new Social Work staff and assists in orientation as appropriate + Daily Coordinator of Utilization Review and Discharge Planning Processes + Rotates with other team leaders in preparing for and leading weekly Clinical High Risk meetings (ensuring room availability, informing department of meeting location, printing current patient lists, etc.) + Implements actions to ensure proactive development and completion of post-hospital plan + Communicates significant increase in work volumes beyond capacity to Social Work Associate Director + Develops, evaluates and revises policies and procedures that guide and support the provision of discharge planning services and are consistent with hospital mission, goals and objectives, state law and regulatory requirements + Vacation/Unexpected Absences Manager + Receives and organizes vacation requests for Social Work staff and determines which requests will be granted + Informs staff of approved or declined requests + Informs staff of vacation coverage and facilitates arrangement during coverage dates (observing work volume to ensure it does not increase above expected range) + Collaborates with Social Work Director to determine vacation requests during high-peak request times (holidays, summer) to ensure fairness and appropriate coverage within pods + Reassigns workload when unexpected absences occur Credentials: + Licensed Social Worker (Required) + LCSW or LSW (Required) + PA License (LSW or LCSW) required. Education or Equivalent Experience: + Master of Arts or Science (Required) + A minimum of 2+ years clinical experience (Required) + 5+ years clinical experience (Preferred) We believe that the best care for our patients starts with the best care for our employees.
