With over 45 years of excellence and more than 200 thriving communities nationwide, we know that strong communities start with strong people, like YOU.If you're ready to use your heart, your skills, and your voice to uplift residents and empower independence, the Service Coordinator role at Shelby Woods Apartments is your next big opportunity!What You'll Do:Empower residents to live independently by connecting them with essential services and support programs.Build and maintain a robust network of local agencies, providers, and wellness resources.Educate and guide residents and families through Federal, State, and local programs tailored to senior adults.Provide general case management including intake and referral services to all residents requesting and/or needing assistance.Refers and links the residents to service providers in the community; for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, healthcare services, preventative health screening/wellness, substance abuse and legal advocacy. Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.
