Job DescriptionFriend Health is seeking a dedicated and experienced Care Manager RN to join our team. In this role, you will play a vital part in developing and implementing a patient care team model that enhances care coordination in alignment with managed care and Patient-Centered Medical Home (PCMH) principles. Our goal is to improve the quality and delivery of comprehensive patient care while ensuring a seamless experience for our patients.Essential Job Duties & Responsibilities:Develop, implement, and evaluate care management policies, goals, and objectivesSupport the care team in care coordination and implementation of patient care plansConduct ongoing clinical assessments and develop integrated patient care plans for high- and medium-risk patientsCollaborate in the selection, development, and maintenance of patient education materialsDevelop individualized care plans based on patient needsWork closely with the supervisor to integrate care coordination services into the team modelMonitor daily activities of assigned staff, assess performance, provide feedback, and develop training and professional growth plansProvide orientation, education, and training for staff as neededAssist with transition of care post-hospitalizationParticipate in integrated care team roundsPerform data analysis and trackingInvestigate and resolve patient and provider complaintsParticipate in relevant departmental and organizational committeesPerform other related duties and projects as assignedEducational Requirements:Current State of Illinois license (LPN, RN, LCSW, or LSW).Bachelors of Science in Nursing (BSN) or related is required.Current CPR certification.Valid Illinois Driver's License.Experience & Related Skills:3-5 years of experience in healthcare care coordination or disease management is required.Active Illinois RN license required.Experience in an ambulatory healthcare setting or federally qualified health center (FQHC) is required.Strong time management and organizational skills in a high-intensity, patient-focused work environment.Demonstrates problem-solving, decision-making, and sound judgment.Exceptional customer service skills.Strong oral and written communication skills.COMPENSATION & BENEFITSSalary range: $86,000- 90,000 AnnuallyBENEFITSHealthVisionDental403bFSA and HSAEmployee Assistance Program (EAP)Occupational Risk for Exposure to TBThis position carries an occupational risk for exposure to Tuberculosis (TB) due to direct client interactions.
